How do I reinstate my 2012 application for 2013 entry?

If you would like to be considered for different programmes, tick the relevant boxes to indicate which choice(s) you want to change. You must also add a new personal statement.

The deadline for submitting this form is 16 November 2012. After this date all old application forms and documents are destroyed, as we do not have the facilities to store them.

The reinstatement fee is due by 31 January 2013. Your application will not be considered until the reinstatement fee has been received.
